InfantSee Exam

Free exam on babies between the ages of 6-12 months. Dr. Matteson only. Infant See exams are done on Wednesdays only. Please fill out the Infant See Form, found below, and fax to (650) 321-2489, mail to 540 University Ave Suite 110 Palo Alto, CA 94301, or email to info@paloaltoeyes.com, prior to the exam.

What is the 20/20 Referral Program?

Your referral of your family and friends is the best compliment you could give us. We want to thank you with the EYECARE TO SHARE 20/20 Referral Program. The 20/20 Referral Program is a program designed to ensure your family and friends have the same comprehensive eye care that you receive from Palo Alto Eyes Optometry. We want to reward our loyal patients for their trust and confidence in us and we love meeting new patients!

What are the Benefits?

As a thank you for your referral, you will receive a $20 eyewear credit for every new patient that you refer. The patient you refer will also receive a $20 eyewear credit.

Is the credit Transferable?

The $20 eyewear credit is NOT transferable between patients.

What can they be Redeemed for?

The $20 eyewear credit can be used towards your NEXT eyewear purchase. They can be used toward your purchase of a complete pair of eyeglasses (frames and lenses), sunglasses, or on an annual supply of contact lenses. Eyewear credits do not have any monetary value.

How do we track 20/20 Referrals?

When a referred patient comes to their first appointment they MUST share with us the referring patient’s name and email address. After getting your name, we will e-mail you a $20 eyewear credit notice. Your credit will be noted in your chart and can be used for your next eyewear purchase or annual supply of contact lenses.

There are no limits as to how many patients you can refer.
